Business Formation & Licensing
Register your business entity (LLC or Corp). Obtain state business licenses and any required state-specific medical equipment dealer licenses. Some states require an "in-state" or "resident" license with extensive applications, proof of insurance, fees, and inspections.

FDA Registration (If Manufacturing/Importing)
If you manufacture, repackage, relabel, or import medical devices, you must register annually with the FDA. Distributors who don't modify products are generally exempt. The annual FDA establishment registration fee applies.
